Annual Update Forms
Each school year all parents must fill out and submit an update form declaring the current health condition of their child. Parents whose child has asthma, diabetes, or insect allergies must also fill out a special form to make us aware of those special needs. For your convenience, forms can be downloaded in Adobe PDF format, 100% compatible with all computers and printers. (You must first have the PDF reader program loaded on your computer. A free reader can be downloaded by visiting http://www.adobe.com/.) To download a form, simply click on the one you need from the Annual Update Forms page found in the Table of Contents to the left of this information. | |||
